Deceased COVID-19 vaccine recipient payments and funeral costs

In cases involving death you may be eligible for payment and support for funeral costs. We’ll make this payment to the deceased’s estate.

What proof you need

Before you can claim, you need to get your doctor to complete a COVID-19 vaccine claims scheme medical report. You need this as proof when you submit the claim. We need your doctor’s responses to help us assess your claim.

You need to provide the following documents with your claim:

  • the deceased’s death certificate or medical cause of death certificate
  • proof that you’re acting on behalf of the deceased
  • proof of any funeral costs and who paid, like receipts
  • proof of any amounts you got or will get from third parties, like funeral insurance.
  • proof of the deceased’s partner and children dependent on the earnings of the deceased at the time of death, if applicable.

How to calculate payments and funeral costs

You need to show your actual costs for the funeral. Remember to deduct any amounts you got or will get from third parties.

If your claim involves death, find out what you can claim in the COVID-19 vaccine claims scheme policy on the Department of Health website.

How to claim

Complete these steps to make a claim under the COVID-19 vaccine claims scheme.

Before you start check who can get it to see if you’re eligible to make a claim under the scheme. You may wish to talk to your doctor about whether you’re eligible.

Claim for someone else

If you’re acting on behalf of someone else for Medicare, you need to submit the claim using all of the following forms:

  • Authorising a person or organisation to act on your behalf – COVID-19 vaccine claims scheme
  • COVID-19 vaccine claims scheme application
  • COVID-19 vaccine claims scheme medical report.

You need to send us your forms and proof by:

  • emailing COVID-19 vaccine claims scheme
  • mailing them to us using the details on the form.

Step 1 – get ready to claim

Before you claim, make sure you also all of have the following ready:

  • proof you were admitted to hospital
  • how you calculated the amount you’re claiming
  • proof to support the amount you’re claiming.

  • using your Medicare online account through myGov
  • using the Express Plus Medicare mobile app.

If you’re using your Medicare online account, sign in to myGov then:

  1. Select Medicare.
  2. Select Make a claim, then Make a COVID-19 vaccine claim.
  3. Follow the prompts.

If you can’t go online or aren’t eligible for Medicare, you can complete the COVID-19 vaccine claims scheme application.

You need to send us your forms and supporting documents by:

  • emailing COVID-19 vaccine claims scheme
  • mailing them to us using the details on the form.

Step 4 – wait to hear from us

Once we receive your claim and all supporting evidence, we’ll try and get back to you as soon as possible.

If you’re claiming more than $20,000, the claim may take longer to assess. This is because we need to consult with others.

We may consult with the following independent third parties:

  • a medical expert
  • a legal expert.

You don’t need to contact us during this time. We’ll contact you if we need more information.

Step 5 – we let you know the outcome

If we approve your claim, we’ll send you a deed to accept. You’ll receive this as a letter in the mail.

Once you accept, we can pay you the amount we’ve agreed on.

If your claim isn’t approved, we’ll write to you to let you know. You can ask us for a review of the decision. Your letter will explain how to do this.
